Athletic Registration Requirements |

Age – 7th grade athletes must not turn 15 before May 1; 8th grades athletes must not turn 16 before May 1. Academic – Athletes are eligible the first year they enter the 7th grade and the first year they enter the 8th grade provided they passed 5 subjects the previous semester. Fitness – Athlete must pass a GHSA approved Physical Evaluation with a proper Clearance Form and complete the accompanying Medical History Form. Registration – All registration forms - waivers, consent forms, permission forms and authorizations must be signed and dated as provided for on the forms. Code of Conduct – Form must be signed and dated by parent/guardian and athlete. Proof of Insurance - Athletes must provide a copy of their insurance card; front and back. |
